As the Area Director for Aveanna’s Concierge Services, Joe brings fourteen years of executive management experience, primarily focused in the private care sector of the long-term care industry. After graduating from The Johns Hopkins University, he quickly learned that his unique specialty was building, leading and educating employees on delivering quality service. As a high energy, enthusiastic business executive, Joe believes in empowering his team to come to their own solutions through hands-on leadership. Joe’s techniques are based around his belief in building and reinforcing the company’s culture and vision. You will rarely find him without a smile on his face and positive words for his staff. He genuinely believes that a happy staff equals happy clients. Happy clients are the ultimate goal for the Concierge Services team.
Having cared for people of all ages in her thirty-eight year nursing career, Ann has experience in a wide variety of home care roles including skilled and unskilled adult home care services as well as formerly co-owning a home care company specializing in services to quadriplegics, brain injury and wound care clients. Her diverse background has given her extensive clinical experience and allowed her to develop a deeply compassionate approach in meeting client’s needs. Ann oversees the clinical department supervising Aveanna’s Concierge Services full-time nursing staff and making sure our team is in line with our CHAP (Community Health Accreditation Program) accreditation.
Tim has served in healthcare for more than 30 years through various roles including pharmaceuticals, population health management, home care, and hospice. Working with people is Tim’s favorite part of his current role at Aveanna. Tim’s strengths include customer service, marketing, operations, and sales which provide the foundation to his passion for enhancing the client experience and care provided by Aveanna’s Concierge Services team.
Elina Polyakov received her master’s in social work from Columbia University and started her career as a clinical psychotherapist assisting individuals and families in need. She was especially drawn to working with the geriatric population finding those interactions very meaningful and rewarding. Elina joined Aveanna Concierge Services, formerly Freedom Home Healthcare, in 2009. She plays a unique role in the company by providing complimentary social work and/or care management services to clients by serving as a liaison between clients, families, and Aveanna. This includes: conducting initial assessments for home care services, assessing client-caregiver fit, providing emotional support to clients and families as clients’ needs change, and intervening during crises. Elina also provides linkages to resources to ensure comprehensive care. Elina works closely with caregivers to provide support and tools to best service their clients. Elina derives enormous satisfaction learning from the clients and caregivers and knowing she is making a difference in their lives.
Rachel joined the Concierge Services team in 2015 after nearly a decade of developing and managing a homecare agency in Essex County. As a social worker, Rachel brings her passion and training for working with clients and their families to ensure they are receiving the care and support they want and need throughout the aging process. In addition to her commitment to quality homecare, Rachel is dedicated to the creation of elder-friendly communities. She has extensive experience in creating and advocating for “aging in place” programs that support individuals and families in the community. Rachel understands that it takes a village to care for our seniors. She loves working with the Concierge Services team and the clients we have the opportunity to serve.
